Virginia State Law requires patients to be referred by a Physician, Physician Assistant, Nurse Practitioner, Chiropractor or Dentist. Upon written referral, patients will be seen only by appointment.

Insurance and Billing:

Williams Physical Therapy will file for all insurance carriers. We are participating Providers for the following:

• Anthem Blue Cross and Blue Shield
• Medicare
• Tri Care
• Med Cost Southern Health
• Gateway Health Alliance
• Workers’ Compensation

As a convenience to you, our office will submit charges for services to your insurance carrier, but you are ultimately responsible for the account. Co payments are required at the time of service, and all patients will be required to establish financial arrangements. Since insurances policies greatly vary, please verify the extent of your coverage with your insurance company. If your injury is the result of a work-related accident, we ask that you supply us with the following information:

1. Name, address and phone number of the employer at the time of the accident.
2. Date of the accident and how it occurred.
3. The employer’s worker’s compensation insurance carrier and address and your claim number.
